Who these terms apply to

In these terms, "we", "us" and "our" refer to Boutique Dental Sydney Pty Ltd, trading as Boutique Dental Bondi, Dr William Choi, associate dentists, oral hygienists, oral health therapists, dental assistants and receptionists providing services at the time of your attendance. "You" and "your" refer to the patient and/or the patient guardian. By attending Boutique Dental Bondi, you agree to these terms and conditions.

Information and consent

We endeavour to inform you about current or potential dental disease or dental problems, diagnosis and prognosis, treatment options, proposed treatment plans, possible side effects, potential accidental damage, costs, and expected treatment timing. It is your responsibility to ask for any information you want before treatment starts. By agreeing for treatment to begin, you confirm that you consent to treatment and have been sufficiently informed about the relevant matters.

Payments

All balances must be paid at the end of each session. If payment is more than 60 days late, a 10% late payment fee will be applied to the outstanding balance.

Any outstanding balance remaining after 60 days may be referred to debt collectors. Collection costs and additional late payment fees may be added to the original balance before referral. If the account is referred to a collection company, any fees and costs incurred must be paid to that collection company.

If treatment has been partially completed and you do not wish to finish it for any reason, we reserve the right to charge you in full or in part.

We reserve the right to change service prices without notice. Prices listed on treatment plans we print and provide are valid for three months from the date of issue. We may change a treatment plan when we recognise that doing so is in your best interest. We will inform you before performing changed treatment, and changes may alter the total cost payable by you.

We may refer you to a specialist when we believe this is in your best interest. We will explain the decision and reasons. You may be charged partly depending on the work completed. We are not liable for specialist costs, and you are responsible for paying the specialist directly.

Deposit policy

A booking deposit may be required to secure appointment times. Deposits must be paid at the time of booking by phone or through the online booking system, otherwise your appointment may not be held. The deposit can be used against your account at the end of treatment or refunded if your health fund covers the cost completely. Refunds can only be made to the credit card originally used.

• $100 deposit for certain appointments, including dental crowns, in-chair teeth whitening, and other procedures subject to change.
• $50 deposit for other appointments, including facial injectables, emergency appointments, check-ups and cleans, subject to change.
• Patients with frequent no-shows or late cancellations may be required to pay a deposit before scheduling future appointments.

Guarantees and products

We guarantee our services, with warranty periods varying by service type. We reserve the right to change warranty periods for individual services at any time without notice.

Services involving dental laboratory products are guaranteed by us, while the product itself is guaranteed by the laboratory. To provide a replacement for a guaranteed product, we may need to redo some procedures that are covered under our service warranty.

We are not liable for any damage or harm caused by dental products, oral hygiene products, or free samples sold or provided to you. We endeavour to ensure product quality but are not liable for possible damage or harm caused for any reason.

Promotions and special offers

Complimentary consultations are for Invisalign, implants and particular cosmetic treatments only. They are not comprehensive dental examinations or treatment appointments. General complimentary consultations may not include X-rays unless the specific offer states otherwise. The free implant consultation offer includes the CBCT scans and planning records needed to prepare an implant quote where clinically required. Special offers are available only after careful evaluation of your teeth and oral environment. We reserve the right to decline treatment that we believe is not in your best interest and to change special offers at any time without notice.

Free implant consultation offer

The free implant consultation campaign is an implant-specific promotional consultation valued at $300. It is not a comprehensive dental examination and does not include treatment. It includes the CBCT scans and planning records needed to prepare an implant quote where clinically required. Suitability, risks, alternatives, timing, inclusions, exclusions, and fees for any treatment are confirmed only after clinical review. The practice may decline or recommend alternative treatment where implant treatment is not considered in your best interest. Payment pathways may be discussed after a dentist recommends a treatment plan and may be subject to third-party provider approval.
